Enrollment Requirements & Procedures

Policy Statement
The College adopts an efficient, fast and systematic registration and enrolment procedure. A student must be officially enrolled in order to receive credit for the course.

Credentials / Requirements
Credentials for Enrollment
Pre-enrolment / Reservation for Freshmen / Evaluation

  1. [Guidance Office] Take the Career Guidance Evaluation.
  2. [Registrar Office] Submit the result of the Career Guidance Evaluation.
  3. [Registrar Office] Fill-up the Pre-enrolment/Reservation Form that will be issued to you.
  4. [Registrar Office] Submit the Form for registration.
  5. [Business Office] Pay the fee(s).
  6. [Registrar Office]Submit the Form to the staff.
  7. [Registrar Office] Get your claim stub from the staff.
New Freshmen Student(s)

Requirement:

  • Take the Career Guidance Evaluation Exam at the Guidance Office anytime before May 2.
  1. Submit your credentials and get your Registration Form from the Registrar’s Desk at the Quadrangle.
  2. Fill out the registration form then submit it to the Registrar’s Desk (for the signature, registration, and enlistment of subject(s)).
    1. Submit filled out form from the Registrar’s Desk to the Assessment Desk.
    2. Obtain an assessment form from the Assessment Desk.
  3. Submit assessment form from the Assessment Clerk to the Cashier and pay the required fees.
  4. After paying the fee(s), go to the Student Affairs Desk to pay the SML fee.
  5. Submit the registration form, assessment, and receipt to the Confirmation Desk.
  6. After confirmation of enrollment, go to the desginated Library Desk for picture taking (School ID and Library Card).
Old Student(s)

  1. Get your grade(s) and Registration Form from your Adviser.
  2. Fill out the registration form then submit it for signing to your Adviser and Dean at the designated area.
  3. Submit the accomplished form to the Registrar’s Desk (for the signature, registration, and enlistment of subject(s)).
    1. Submit filled out form from the Registrar’s Desk to the Assessment Desk.
    2. Obtain an assessment form from the Assessment Desk.
  4. Submit assessment form from the Assessment Clerk to the Cashier and pay the required fee(s).
  5. After paying the fee(s), go to the Student Affairs Desk to pay the SML fee.
  6. Submit the registration form, assessment, and receipt to the Confirmation Desk.
  7. After confirmation of enrollment, go to the designated Library Desk for picture taking (School ID and Library Card).
Transferee(s)

Requirements:

  • Evaluation of Certification of Grade(s)/OTR in the Registrar’s Office.
  • Present result of evaluation to your respective Dean.
  • Take the Career Guidance Evaluation Exam at the Guidance Office.
  1. Get a Registration Form from your Adviser.
  2. Fill out the registration form then submit it for signing to your Adviser and Dean at the designated area.
  3. Submit credential(s) and the accomplished form to the Registrar’s Desk (for the signature, registration, and enlistment of subject(s)).
    1. Submit filled out form from the Registrar’s Desk to the Assessment Desk.
    2. Obtain an assessment form from the Assessment Desk.
  4. Submit assessment form from the Assessment Clerk to the Cashier and pay the required fee(s).
  5. After paying the fee(s), go to the Student Affairs Desk to pay the SML fee.
  6. Submit the registration form, assessment, and receipt to the Confirmation Desk.
