Student Affairs Office

The Office of the Student Affairs is headed by Mrs. Melba C. Ano and is coordinated with Mr. Noel Sumingwa in overseeing activities conducted by clubs, student body organizations, advisers as well as officers and members of the said group.

Scholarship grants and student assistance services are also accorded to this department.

The Sports and Athletics Committee reports under the same umbrella of authority.

The SAO is likewise the office which appropriates proper decorum and manages student discipline throughout the campus.

Organizational Structure of the Student Affairs Office
Procedures – Lost ID
ID-Request-Slip
The following procedures are applicable to students who have lost his / her I.D. card.

  1. The student shall be given (3) days to search. While waiting for the three days, he will be given a temporary gate pass.
  2. After 3 days he will be given request slip for ID Replacement. The request shall be approved by the dean then pay necessary charges at the cashier.
  3. Back to the Office of Student Affairs and present payments to the Dean of Student Affairs.
  4. Student shall sign in the logbook.
  5. Concerned student will claim his new ID in the library.
  6. In the case of forfeited ID he/she will immediately make a request letter then forward to the concerned Dean for approval.
  7. Then pay necessary charges to the Cashier.
  8. Student shall sign in the logbook.

Procedures – Call Slip
Call-Slip
The following procedures are applicable to parents / visitors who wishes to see a student during the student’s vacant period.

  1. The Dean of Student Affairs shall receive request to call student in the campus either by the parent/visitor and issues a call slip.
  2. The Dean shall issue a call slip herself or can authorize somebody to issue the call slip to the concerned student.
  3. The student presents him/herself to the Office of the Student Affairs during their vacant period.
  4. Student/s and visitors/parents can talk inside the office.

Procedures – Admission Slip
Admission-Slip
The following procedures are applicable to students who have missed an activity / activities while he / she is absent.

  1. The Dean of Student Affairs shall receive request letters or parents letter indicating why and when the student was absent.
  2. The Dean of Student Affairs shall issue the admission slip to the student.
  3. The student shall fill up the form indicating whether to request admission slip, claim class cards or both.
  4. The dean shall approve or disapprove the reason of absence if excused or valid the student is given consideration for any missed activity during his/her absence. If unexcused the student is not given any consideration for any missed activity during his/her absence.
  5. The Dean of Student Affairs returns the admission slip to the student.
  6. Student shall sign in the logbook.
  7. Student to present admission slip to the teacher and re-enters the class.

Procedures – Claiming of Dropped Class Cards
The following procedures are applicable to students whose class card is dropped at the Student Affairs Office.

  1. The Dean of Student Affairs shall receive requests and issue the Admission slip.
  2. The student shall fill up admission slip then returns it back to the Dean of Student Affairs.
  3. The student shall fill up the admission slip indicating to claim class card then returns back the admission slip to the Dean of Student Affairs;
  4. The dean issues back the class card to the student;
  5. Student shall sign in the logbook.
  6. Student to present class card to the teacher and re-enters the class.
